Intensive Care Coordination (ICC)

We Care provides Intensive Care Coordination (ICC) to help children and their families navigate significant mental health and behavioral challenges. Designed for children requiring extra support—including those in foster care or reunification—our program ensures that families have access to the right services, professionals, and resources in a coordinated and compassionate way.

Our Intensive Care Coordinator (ICC) works closely with families through Child and Family Team (CFT) meetings, creating a collaborative plan that prioritizes the child’s well-being and stability. Families are at the center of every decision—we listen, support, and empower caregivers to be active partners in their child’s care.

How We Support Families

  • Personalized Care Coordination – A dedicated ICC facilitates ongoing meetings, ensuring children receive the services they need.
  • Service Linkage & Referrals – We connect families to mental health care, educational support, behavior management strategies, and skill-building resources.
  • Ongoing Collaboration – We monitor progress, adjust treatment plans as needed, and ensure seamless communication among all care providers.
